    Compassionate Care Givers Inc. provides supports coordination services in the following counties: Atlantic, Bergen, Burlington, Camden, Cape May, Cumberland, Essex, Gloucester, Ocean and Salem. We provide the best in supports to individuals with developmental, physical, emotional and mental challenges through innovative community supports.

    Using the person-centered planning approach, our Supports Coordinators assist individuals in the development and implementation of Individualized Service Plans (ISPs). Assisting individuals in gaining access to needed programs and State planned services in obtaining entitlements, medical, social, educational/vocational and other services; coordinating with other community providers that would best serve those needs.

    Our Supports Coordinators work with each individual as a new and separate case without assuming any similarities in facts and circumstances. As part of the process of getting to know someone, our Support Coordinators work with the individual, his or her family and the entire support team to identify the roles, activities and relationships that are important in their life and based on this knowledge, the Coordinators will know what community resources are going to be most important to the individual.

    What We Do As Your Supports Coordinator:

    Supports Coordinators assist individuals with disabilities and their families to develop plans to find the most appropriate services and select the most appropriate service delivery model, based on the individual person’s needs and wishes.

What Is Supports Coordination?



DDD works closely with Supports Coordinators to monitor the quality of the services they provide. At all times, it is important for a person or family to know that they get to choose who works for them as their support coordinator. DDD will help when it comes time to choose a support coordinator.

In all cases, the Supports Coordinator is the first and best option to help a family determine what services are available to best suit a person’s needs.
